We want your help in exploring what the issues are in community-based specialist palliative care and coming up with possible solutions.

Over the coming months we will have lots of ways that you can get involved and opportunities to give your views.

The survey is also available as a fillable pdf.  To complete the survey please download/save it and then open in Adobe Acrobat Reader or Foxit. Once you have completed the survey please press submit at the end of the survey and it will automatically be submitted. Alternatively, you can complete the survey in your website browser and download/save it with your responses and email it to nhsnwlccg.endoflife@nhs.net.  You can also return using the address below.  

Your responses will be analysed anonymously, and it is important that you are as honest as possible to allow us to hear things we otherwise may have missed or ideas we may not have thought of. We will use the feedback provided to monitor and improve the services we commission.
